Offered at the Floyd County Public Health Office, 1003 Gilbert Street, for children up to age 18 that have Medicaid (T19) insurance, that are under-insured, or have no insurance. You MUST set up an appointment IN ADVANCE by calling (641) 257-6111, as clinics do fill up. If you do not have insurance, you will be asked for a $19 per shot donation.
